** Special note for AOL Users

To use the forms you will need the Free Acrobat Reader which may be downloaded from their website [simply click on the hyperlink above and follow their instructions. This download may take several minutes depending on the speed of your system.].

These forms were created using Adobe Acrobat 5.0. This format allows you to easily view, edit and print our forms from their PDF files. The quality of the printed form will be the same as the original.

Instructions:

  1. Choose the form you wish to use from the left navigation menu bar. Acrobat Reader will open automatically to display it, and you should see the form on your screen. This may take several seconds depending on you internet speed/bandwidth.

  2. Once the form is on your screen you have several options. You may print the blank form if you wish or you may edit the form [fill in the information required] on screen and then print the edited version.

  3. To fill out the form online, simply click on the form at the point you wish to enter information. For example, click on the space for "Class" and a cursor will appear, simply type the class name as it should appear on the printed form. The checked and highlighted portions below are the required fields.

    CHECK BOXES: The check boxes on the forms are configured to automatically be checked when you click on the box, there is no need to enter an 'X' or other mark in these boxes.

    DROP DOWN MENUS: The form also utilizes drop down menus for the instructors' title (Professor, Instructor, Dr. etc.) and also for the cover color choices. Please highlight the appropriate item as you would in any other windows based system to make your selection. You may also edit the text if the desired choice is not found in the menu.

    To move to another space on the form you may either click the mouse on the desired space or you may tab through the fields on the form. Should you make a mistake in typing, you may return to any space to make corrections by simply clicking the mouse on the desired space.

  4. When you are satisfied with the edited form simply print the form [choose the 'Print' option from the 'File' menu or click on the 'Print' icon].

IMPORTANT: You will not be able to save the edited version of the form in Adobe Acrobat Reader. Please print the form and review it carefully for errors before going on to the next form or exiting from the program. [If you have the full version of Adobe Acrobat (not the free Acrobat Reader), you will be able to save your edited form on your computer.]

NOTE TO AOL USERS: While AOL's proprietary browser does support the Adobe Acrobat Reader, there is a technical problem within this browser that prevents it from displaying PDF files online. However, if you download the PDF file to your hard drive, you can then open it with Acrobat Reader offline. You may also consider downloading another web browser, such as Netscape Navigator 3.0 or higher or Internet Explorer 3.0 or higher (instead of using AOL's web browser) to view and edit the PDF files via the web.
